About this Form

Form number: 270-2b

Audience:  MCPS Staff

Last revised: September 2017

A Section 504 evaluation is to be conducted by the Section 504 school-based team for general education students (students who do not have Individualized Education Programs) who may have a disability and be eligible for accommodations under Section 504 of the Rehabilitation Act of 1973, which prohibits discrimination on the basis of disability. The Section 504 school-based team must be composed of persons knowledgeable about the student, the meaning of evaluation data, and placement/accommodations options.

All Section 504 information should now be documented using the online Section 504 platform. If you still need a copy of previous forms due to extenuating circumstances, please contact the MCPS Resolution and Compliance Unit at (301) 517-5864.
