This form should be used to request any modification to a high school transcript after 45 days of the end of the marking period. An appropriate rationale must be provided and the counselor and principal must recommend the modification before submitting to the Director of Learning, Achievement and Administration. A copy of MCPS Form 355-27C is provided to the teacher and school counseling office to retain, according to established procedures.
