This is a sample of the SR-1, Student's Personal Data card, and is for reference only. Schools should complete a SR-1 card for each student on the green card stock version of this form ordered through the Department of Materials Management.
A Student's Personal Data Card (SR-1) should be created for ALL students entering MCPS, regardless of age. Information maintained on the card should be accurate, readable, and current.
