INSTRUCTIONS:  To be completed by the trip sponsor and submitted to the director in the Office of School Support and Improvement (OSSI) by May 1. Trip must be approved prior to parent/guardian notification of the proposed trip. See MCPS Regulation IPD-RA, Travel-Study Programs, Field Trips, and Student Organization Trips.
