INSTRUCTIONS:
1. Complete this fillable form to submit invoices for reimbursement for purchases of instructional supplies, food supplies for FACS classes, postage, textbooks under $50.00, and medical supplies under $25.00. All invoices tendered for reimbursement must have been paid by the school.
2. Email requests for reimbursement from non-school accounts to appropriate account owner for approval. Account Owner should email approved reimbursement and proper backup documents to the Accounts Payable drop box, accountspayable@mcpsmd.org.
3. The Name of the Payee, Payment Date, Payment Number (the check number or iPayment receipt number) and amount to be reimbursed must be on the school reimbursement form.
a. If the payment was made with a school check, please include a copy of MCPS Form 280-55, Independent Activity Funds (IAF) Request for a Purchase, copy of the check, and the original itemized receipt(s) or invoice(s). Include check number on receipt.
b. If the payment was made through iPayment, please include a copy of MCPS Form 280-55, Independent Activity Funds (IAF) Request for a Purchase for the iPayment payment, the iPayment receipt, the iPayment invoice (circle the invoice lines to be reimbursed) and the original itemized receipt(s) or invoice(s). List each line on the reimbursement form. For reimbursement for payroll, list the employee’s name, the date worked, the total amount paid and the employee’s timecard (one page view) or Form 280-46, Independent Activity Funds Request for Payment to MCPS Employees for Services (Work) Performed.
4. This reimbursement form is due to the Department of Controller office by the 10th of the month following the date of the IAF expenditure.
5. Reimbursement form should only contain invoices paid within the same calendar month. Give each reimbursement form a unique date. Scan all documents in the portrait format; all documents must be legible.
