DIRECTIONS:  This form may be submitted with one copy of MCPS Form 280-49A to authorize and pay multiple individuals with Independent Activity Funds (IAF) to perform the same/similar work for a project within a specified period of time - only if the amount due to the individual(s) for the project does not exceed $1,500.00, and if all payments are changed to the same account. A separate MCPS Form 280-49A and purchase order is required for all vendor/contractor payments that exceed $1,500.00.
