INSTRUCTIONS: This Agreement specifies the rights and responsibilities of the Sponsoring Organization (SO)
and the Family Child Care (FCC) Provider as participants in the Child and Adult Care Food Program (CACFP), in
accordance with CACFP regulations 7 CFR 226. The CACFP is administered by the Maryland State Department of
Education (MSDE) and funded by the U.S. Department of Agriculture (USDA).
This Agreement must be completed and signed by the SO and by the provider. The provider and the SO must keep a copy of the signed Agreement.
