High School Extracurricular Activity Summer Athletic Practice—Employee Eligibility Verification

Instructions: This form is used to report to ERSC the names and ID numbers of fall athletic coaches/sponsors who will begin work with athletes in August. Complete all information and submit to ERSC by July 15. Enter TBD for positions not filled by July 15 and submit updated roster as soon as coach/sponsor is identified. ERSC will verify employee eligibility prior to the start of summer practices. Work may not begin until ERSC employee eligibility verification is complete and the school is notified.
